Abstract
As long as there is no consensus on the definition, diagnostic criteria, and pathogenesis
of left ventricular hypertrabeculation (LVHT)/noncompaction misdiagnosis, overreaction,
and mistreatment of this abnormality will eventually continue. When considering a
therapy for patients presenting with LVHT it has to be taken into account that LVHT
is not associated with an increased risk of thrombo-embolism in the absence of atrial
fibrillation or systolic dysfunction. There is also no proof that the degree of LVHT
correlates with the degree of systolic dysfunction. Secondary prevention of stroke/embolism
and sudden cardiac death is indicated in LVHT only if cardiac investigations reveal
atrial fibrillation, systolic dysfunction, or ventricular arrhythmias. Up to what
we know at the moment LVHT patients should be closely monitored by conventional cardiologic
and neurologic diagnostic tools and appropriate measures taken, only if indicated.
